This full-time position involves providing clinical and outpatient services for high-risk older adults in subacute and long-term skilled nursing facilities. The successful candidate will be part of a primary care team dedicated to holistic care, including acute, chronic, preventive, and palliative care services. Responsibilities include comprehensive evaluations, diagnostic tests, prescribing treatments, and patient counseling._x000D x000D_Location:_x000D_Austin, MN, 40 miles from Rochester, MN._x000D_This role involves travel within the Southeast Minnesota region._x000D x000D_Key Aspects:_x000D_Collaborative work with advanced practice providers._x000D_Balance of subacute and long-term care patients._x000D_Medical directorship opportunities._x000D_Blend of primary outpatient internal medicine or family medicine, and senior services care delivery._x000D_Access to health system specialty consults, pharmacy, and social work._x000D_Mentorship, coaching, education, and research opportunities._x000D x000D_Requirements:_x000D_Board Eligible or Board Certified in Internal Medicine._x000D_Geriatric Fellowship is highly desirable._x000D_Must have or be able to obtain a Minnesota Medical License._x000D_Must possess an M.D., D.O., or equivalent degree._x000D_Clean professional history._x000D x000D_Benefits:_x000D_Full-time, exempt status._x000D_Benefits eligible._x000D_80 hours per pay period._x000D x000D_Note:_x000D_Austin, MN offers a vibrant community with a rich quality of life, providing a harmonious blend of urban and natural settings.
