Job Quick Facts: --- Specialty: Geriatric/Internal/Family Medicine --- Job Type: Permanent --- Facility Location: Wilmington, DE --- Service Setting: Inpatient/Outpatient --- Reason For Coverage: Vacancy --- Coverage Period: ASAP - Ongoing --- Coverage Type: Clinical + Call --- Work Schedule: 8a to 5p --- Call Schedule: After 5p & Weekends; Rotation --- Patient Demographics: - Geriatric patients or 55 yrs or older. --- Duties: - Be part of or join the Program of All-Inclusive Care for the Elderly (PACE) organization. - Provide nursing home care. - Make patients able to live safely in the community at the time of enrollment and live in a PACE service area. - Provide the entire continuum of care & services to seniors w/ chronic needs while maintaining their independence in their home for as long as possible. - Carries out medical procedures as indicated. - At the direction of the Medical Director, reviews medical outcomes/statistical & utilization data, including reviewing of necessity of hospitalizations and use of specialists. --- EMR: TruChart Requirements: --- Active DE License --- BC in Geriatric Med (Accept IM/FM) --- BLS --- DEA --- CDS/CSR --- At least 1 year of experience working w/ frail or elderly population --- EMR TruChart experience (preferred) Benefits: --- Competitive Salary (Starting at $208,000) --- Health, Dental and Vision insurance --- Short and long-term disability --- 403b --- Generous PTO --- Mileage reimbursement
