The Substance Use Recovery and Empowerment (SURE) program provides individuals with support and assistance in developing a plan for recovery from substance use and related mental health concerns. The team’s goal is to assist those diagnosed with a substance use disorder with achieving long term recovery while living a fulfilling and productive life. Qualifications: LSW or LAC 3 years of experience in addictions counseling Valid Driver's License in good standing Responsibilities: Assesses consumers and their counseling needs using the ASI or other standardized clinical interview tool, and diagnosing clients for substance use and mental health disorders using the DSM-5 Determines the appropriate level of care according to ASAM Criteria Obtains previous records that are relevant to the current treatment episode Unless clinically contraindicated, collaborates with the consumer to develop a client centered, recovery oriented written treatment plan that includes goals and measurable objectives. Provides the substance abuse and mental health counseling services specified in the consumer treatment plan Reviews consumers throughout the treatment episode according to the ASAM Criteria, in order to determine the need for continued services, transfer, or discharge/transfer. Reviews and, where necessary, revises the consumer treatment plan to address ongoing problems. Develops a consumer discharge/transfer plan to ensure movement to the appropriate levels of care Contacts, consults and coordinates with referral sources, as needed. (eg. mental health treatment providers, criminal justice agencies, schools, employers, DCPP). Participates as a member of a multidisciplinary team for assigned clients Provides active case consultation.
