POSITION:Clinical Director DEPARTMENT: NursingSUPERVISOR: Executive Director (first) and Regional Director of Operations and ClinicalServicesQUALIFICATIONS PREFERRED: Education: Registered Nurse or Licensed Practical Nurse in good standing with theKentucky Board of Nursing. Experience: Preference in the field of geriatrics either through training or experience. Priorsupervisory duties in: assisted living, home health, or nursing home preferred.Working conditions: Assisted living and office environment; light lifting; irregular hours;routine engagement with residents, staff and visitors.SUMMARY OF DUTIES:1. Organize and recruit clinical service staff and scheduling for 24-hour coverage to meet allresidents’ needs.2. Subsequent to hire: training, scheduling, supervising and ongoing evaluation of clinicalservice staff for all shifts, delegating as necessary.3. Establish and maintain goals and objectives for quality resident care consistent with facility goals and objectives. 4. Assure that the standards of practice are of the highest quality consistent with thestandards of professional practice and maintain compliance with the Assisted Living andDementia Care regulations in the State of Kentucky.5. Ensure person-centered Care Plans are initiated and updated as needed and signed off by theRegional Clinical RN.6. Review residents’ functional needs assessment on admission, yearly and as needed to ensurean appropriate care plan is in place and communicate the changes with family and executivedirector.7. Plan and coordinate services affecting resident care while working cooperatively withother departments and clinical service to provide the highest quality care to residents.8. Maintain good relationships with residents and their families, clinical service staff and thecommunity.9. Assure adequate 24-hour service coverage in the facility each day.10. Assess and resolve personnel issues and build in the program maximum associatemotivation. Ensure the environment is free of drama and gossip. Reprimand and educate asneeded. For our residents to feel safe and happy, staff must have good attitudes.11. Participate in the admission of all residents including functional needs assessment for properplacement. This may include home visits and medical history review.12. Assess and maintain adequate stock of supplies and equipment within the approved budget.Understand the budget process and adhere to the budgeted amount of spending monthly. If forany reason the targeted budget amount will be more than budgeted, get approval from theExecutive Director prior to spending.13. Establish and maintain protocol for resident care planning within specified time framesand modify when necessary.14. Conduct resident care conferences with families and physicians upon any change ofcondition and as needed.15. Coordinate discharge planning as needed.16. Conduct all necessary in-service training and associate meetings that may be required.17. Prepare ongoing reports as directed by the Executive Director.18. Other duties that may be assigned from time to time by the Executive Director.19. On call 24/7 for immediate needs of care staff and residents to provide guidance.20. Review and report all resident and staff incident reports with Executive Director.21. Communicate any incidents of concern to the Executive Director.22. Ensure resident call lights are being answered timely.23. Stay up to date with state regulations.24. Respecting HIPPA laws and maintaining confidentiality.25. Provide education to staff to ensure all job duties are understood and being carried outappropriately. Writing up any staff that break the rules as listed in the handbook andensure they understand that continued unsatisfactory performance could result intermination.26. Understand all job duties and communicate with the supervisor if you do not. You will beheld accountable for those job duties and subject to reprimand if they are not performed astrained.27. Good communication, keeping the Executive Director and other key staff informed ofwhat is happening and needed in memory care.28. If guidance/assistance is needed beyond the Executive Director, contact the RegionalDirector of Operations and Clinical Service for guidance.29. Being present during surveys and unannounced state visits to provide information needed andunderstand any concerns or recommendations and make sure they are in place as directed.30. Review, understand and share best practices with staff as part of their training.31.
