Description:
Full time (35 hours) providing medication management to a child & adolescent panel 15 minute med checks with 45-60 minute psychiatric evaluations, 18-20 patients per day Provide clinical care (medication management/psych evaluations) for patients to support efforts to achieve care plan goals Ensure appropriate transitions of care with external Article 31 providers, hospitals, etc. Reviews case records to monitor quality of care Community health setting, full age range of patients, diverse caseload
