J ob Summary: The certified coder is responsible for the assignment of all coding classifications to patient encounters based on documented diagnoses and procedures. The coder will perform regular coding audits and reviews of services provided in outpatient clinic sites, with the purpose of identifying opportunities to improve knowledge and practices around code entry, clinical documentation, risk adjustments, and overall regulatory compliance. Upon chart reviews, the coder will ensure documentation is complete, accurate, and specific and will follow-up with providers to obtain clarification (and provide education) as needed. This role will consistently partner with clinicians to support education, understanding, and overall improvement around diagnostic coding practices, ensuring both documentation and coding provide a clear representation of the complexity and severity of patient illness(es). This role will ensure applicable laws, regulations, and industry standards are adhered to, including confidentiality of patient information as well as complete and accurate record-keeping. The coder will ensure CMS regulations for coding and documentation are met, including unique RHC billing guidelines and requirements. This role will also prepare regular reports for clinic leadership to show trends and summaries of coding accuracy and areas of opportunity. Performs other department functions as assigned. Education/Experience/Minimum and Preferred Qualifications: High school graduate or equivalent. Excellent customer service skills. Excellent written and oral communication, including grammar and punctuation Certified Coding Certificate (CPC, CCS-P, CRC, CEMC, COC, CCS or CCA) required. Proficient in ICD-9/ICD-10-CM and CPT coding principles. Must have knowledge of Medical Records terminology. A minimum of (2) year as a coder in clinic setting preferred. Rural health clinic experience preferred. Epic experience preferred. BLS certification within 6 months (company provided). Ability to interact with all levels of the organization Understanding of adult learner principles Able to communicate complex concepts and motivate other team members Job Responsibilities: Electronically assign timely coding of diagnoses and procedures for inpatient and outpatient accounts, using ICD-10, CPT, and HCPCS coding classification systems to meet billing system requirements. Work closely with physicians, technicians, Patient Financial Services, and other integral parties to uncover and discuss coding analysis results. Develop, modify, and execute company policies and procedures that affect immediate operations and may also have an organization-wide impact. Ensure records are coded within a time frame recognized as standard. Review charts thoroughly to ascertain all diagnoses/procedures. Contact responsible physician in a professional, tactful manner, if diagnoses is not available on chart. Include Coders' diagnoses and procedures on clinical summary agree with physician's preference 100% of the time. Refer chart to clinic leadership if there is any question regarding diagnoses/codes. Utilize computerized coding and abstracting equipment. Deal with a wide variety of personalities in a professional and tactful manner. Display a willingness to support the policies and procedures of the facility and department. Observe confidentiality and safeguard all patient-related information. Perform performance improvement functions through data collection and documentation review. Maintain a good working relationship within the department and with other departments of the facility. Stay current in field through continuing education, professional societies and journals. Review regularly department and hospital policies, performance improvement documentation, and accrediting organization and state requirements. Support and maintain a culture of safety and quality.
