Visiting Nurse Association of Southeast Missouri is looking for a Central Intake Nurse/Scheduler to join the VNA Home Health/Hospice team. VNA has recently celebrated 50 years in the Home Health industry. We have a great Home Health/Hospice team and would love for you to be a part of it. If you are looking for an opportunity to use your nursing skills to become a resource to help our nursing and therapy staff, please review this job posting and take time to apply, we look forward to talking to you. TYPE OF WORK INVOLVED: Know and understand the eligibility details of all service programs. Responsible for all incoming referrals, eligibility screening, coordination and compilation of clinical information and scheduling visits. Develop relationships with Discharge Planners and Community Leaders, meeting with them at least bi-annually. Monitor Referrals, i.e. by referral source, payer source, and track/trend. This information should be shared with Marketers within the week should a certain trend occur. Assist in answering central intake phone calls as needed. Schedule Home Health nursing visits and Physical, Occupational & Speech therapy visits. Prepare weekly schedules for HH nurses. Adjust schedules daily POSITION REQUIREMENTS: Certification: Currently licensed as a Licensed Practical Nurse or Registered Nurse in the State of Missouri Experience: At least one year of experience in a healthcare field functioning in the role of an L.P.N or R.N. *Additional responsibilities and qualifications will be available at the point of the interview.
