Collaborate with the health care team to assess, plan, implement, monitor and evaluate options and services to enable the progression of patients across the care continuum. Responsibilities : Coordinate the case management process. Coordinate the discharge planning process. Perform utilization review duties. Support PHIP. Maintain documentation. Provide information regarding denials and approvals. Provide education to patients, physicians, multidisciplinary team & the community. Requirements : Graduate of accredited school of nursing. Two years of recent nursing experience in an acute setting. RN License in the state of Illinois AHA CPR certification
