Back Bay Mental Health is opening a new IOP Behavioral health practice right on Commonwealth Ave. We are known as being one of the best employers in community because we care for our staff and treat them like family. We are looking for dynamic individuals who want to work in a growing and vibrant company. Currently we have a need for multiple Case Managers. This is a M-F in-office position. Previous experience with mental health/behavioral health treatment and case management strongly preferred. Traditional case management functions will include but not be limited to: · Developing the client discharge/transfer plans to ensure movement to the appropriate levels of care. · Contacting referral sources, providing case consultation and coordination with referral sources (for example: mental health treatment providers, criminal justice agencies, schools, employers, Child Protection Services) · Coordinating ongoing care for client housing and therapeutic needs with the Back Bay Treatment team, substance abuse treatment providers, healthcare providers, community resources, and family members · Participating as a member of a multidisciplinary team for assigned clients · Documenting all counseling and education services, assessments, reassessments, referrals and follow-up in the client's clinical record, providing appropriate signatures and dating of such entries, including those made in electronic records · Positively promoting Back Bay Treatment to prospective clients and employees · Orientating new clients to the facility and the PHP/IOP facility · Facilitating communication with ancillary services · Attending clinical and administrative meetings to integrate inter-departmental sharing of information · Building positive relationships with clients · Demonstrating adaptability and flexibility to changes in the work environment · Following Infection Control Guidelines at all times · Reporting personal symptoms of suspected illness or contagious diseases to the Infection Control · Providing a safe, therapeutic environment for staff and clients. · Case Managers may also perform the work of a Counselor including: 1. Obtaining previous records that are relevant to the current treatment episode 2. Providing active case consultation 3.
