Position Case Manager Description This position provides case management services to promote recovery and wellness in adults with severe and persistent mental illness. Our Case Managers work as an integral part of an inter-disciplinary team to provide community-based supports, including resource development, promoting good health, crisis intervention and treatment planning for successful goal attainment. Position Requirements Minimum of AA/BA required; LSW/LPC, CCDCII or II strongly preferred. Experience with this population preferred.
