Case Manager

Description:

Telephonically (remotely) assessing, planning, implementing and coordinating all case management activities with patients to evaluate the medical needs of the member to facilitate the member's overall wellness. Develops a proactive course of action to address issues presented to enhance the short and long-term outcomes as well as opportunities to enhance a member's overall wellness through integration. Utilize assessment skills and discretionary judgment to develop plan of care based upon assessment with specific objectives, goals and interventions designed to meet member's needs and promote desired outcomes Coordinate services between Primary Care Physician (PCP), specialists, medical providers, and non-medical staff as necessary to meet the complete medical needs. Provide patient and provider education. Requirements: Holds a Compact Registered Nurse License and should be residing within Florida, Indiana, Missouri, or Tennessee. 3-5 years of hospital Medical/surgical experience. Associate Degree or nursing diploma required. Requested criteria above average computer skills (Microsoft Office suite). Preferred: Triage, case management and/or rehabilitation experience preferred.
