We accomplish this by embodying a patient-centric culture while supporting positivity and the growth of our employees both professionally and personally. -As a Case Manager, you will monitor and manage patient care to promote continuity of care, optimal patient outcomes, patient satisfaction, cost efficiency, and compliance. You will be part of a robust and caring multidisciplinary team caring for our patients from admission through transfer or discharge.Required Skills: --- - -Requires critical thinking skills, decisive judgment and the ability to work with minimal supervision. --- - -Must be able to work in a stressful environment and take appropriate action. --- - -Demonstrates proficiency in verbal and written communication skills. --- - -Excellent interpersonal skills and the ability to interact effectively and professionally with individuals at all levels; both internal and external.Position Requirements: Associate's degree in nursing required Bachelor of Science in nursing preferred Current State of MI RN license required 1 year of case management experience preferred Basic Life Support (BLS) certification required Annual influenza vaccination or declination requiredPosition Details: .001 FTE (PRN, variable hours per week) 8 hour shiftsEqual opportunity and affirmative action employers and are looking for diversity in candidates for employment: Minority/Female/Disabled/Protected Veteran
