Case management will have a therapeutic component and evidenced based practices should be utilized. Participation in county col l a b orati v e m ee ti n g s to id e ntify se r v i c e gap s , b arri e r s, a n d s ucce s s e s in or d e r to create b es t practi c es , p oli c i es , p roc e d ur e s, a nd p rotocol s . Pro v id e tr a ining and outreach a nd i d e ntif y a nd s e e k part n e r s hi p s with ot h e r a g e nci e s a nd orga n izations for coll a boration. Qualifications BA in social w ork or r e lat e d f i e ld required Masters and BBS registered intern preferred. A minimum of 3 y e ars r e l e v a n t e xp e r i e nce r e quir e Exp e ri e nce w orking w i th t e e ns a nd/or y oung adults i nclu d i ng tr a n s itional age y out h , spec if ically f o r me r f o s t e r a n d probation y o u t h pr efe rr ed. Specific Activities Duties include but are not limited to: Utilize engagement strategies to build rapport, identify strengths and barriers to participant self-sufficiency, and empower youth to achieve goals and permanent independence. Provide intensive case management services d es i gn e d to b uild a n d su p p o rt participa n t s ' r e latio n s hips w ith fami l y a nd co m m unity with a n em phas i s on p e r m a n e nt co n n e ction Utilize evidenced based practices to engage youth in participation of services that are client-centered, culturally appropriate, trauma-informed, in order to improve the short/long-term health, safety, and well-being of youth. Utilize evidence based practices as the modality for crisis management and safety planning, as well as individual and group sessions. Provide coordination of care with other mental health professionals, primary care providers, and other supportive services. Coordinate and engage youth in activities in the community to stimulate alternative interests, improve emotional and physical wellbeing, foster leadership development, enhance self-esteem, and support personal growth and satisfaction. Assess appropriate applicants and co-create an individualized case plan, monitoring the case plan and providing assistance as needed. Collaborate w ith County ILS w ork e r s , ILS co n tr a c tor s , social w ork e r s , p robation o ff ic e r s , a nd ot h e r r e quir e d parties in d e v e lo pm e nt a n d de liv e ry of s e r v ice p la n s . Participate in multi-disciplinary teams, serving as an expert on matters related to behavioral health. Uphold LifeSpring program compliance and contractual standards through direct monitoring of participants. Ability to work independently and use sound judgment in decision making. Complete all other duties as assigned. Schedule/ Compensation FT/ nonexempt position Mon-Fri 8 hours a day may include evenings and weekends. Evening and weekend on call shifts MA or BA+5- [$21.00 - $26.64 ] MA+BBS [ $26.64 - $30.00] Salary will commensurate with experience and education. Differential pay offered for bilingual skills. EEO.
