Case Manager- Intensive Case Management
BASIC PURPOSE OF THE JOB The primary focus is to assist clients with functioning in the least restrictive environment. The case manager is responsible for assessing, linking, monitoring, and advocating for all assigned clients. The case manager assures that all assigned clients received the most appropriate services available to meet their identified needs. REQUIRED KEY FUNCTIONS: 1 To provide assessments, treatment planning, discharge planning, care decisions, and reassessments. 1 Is compliant with and follow 65-E15, SAMHPO, AOP standards, complete Person to Person training and AHCA regulations. 1 Responsible for maintaining units of service (UOS) expectations 1 Completes all documentation and billing of all service provision 1 Is required to work in office, including field work (working with clients in their homes) MIN EDUCATION: Min of a Bachelor's Degree in a Human Services related position with at least 1 yr background with children and families in need of services. REQUIRED SKILLS, KNOWLEDGE AND ABILITIES: BA/BS degree in social work, psychology, or Human services related position. To perform this job successfully , an individual must be able to perform the essential duties satisfactorily. Background in community health preferred.
