Job Title: Care Management Supervisor Location: Newark, DE Shift: Day Shift Salary: $92,000.00 - $124,000.00 Job Summary: We are seeking a dedicated and experienced Care Management Supervisor to oversee our Case Management & Social Work staff. This role involves direct care management services to a defined patient population and providing supervisory services to the team. You will work closely with department managers, directors, and vice presidents to guide and educate on all phases of the care management process. Responsibilities: Supervise day-to-day operations of the Care Management team, ensuring effective service delivery. Formulate discharge plans that provide patients with appropriate post-acute care services. Assess patient needs, mental status, family support systems, financial resources, and available community and governmental resources. Develop and implement comprehensive care management plans. Oversee staff attendance, including PTO, disability time, and FMLA, to maintain optimal staffing levels. Provide orientation, mentoring, coaching, and evaluation of department staff. Coordinate staff training and development programs. Participate in budget preparation and monitoring in collaboration with department leadership. Engage in performance improvement activities and compliance with regulatory guidelines. Manage and prioritize incoming requests and needs, escalating to management as necessary. Qualifications: Master's Degree in Social Work (MSW) required. Licensed Master Social Worker (LMSW) or Licensed Clinical Social Worker (LCSW) in Delaware preferred, required by June 11, 2024. Minimum of 5-7 years of experience in a health care environment, with at least 2 years in a leadership role. CCM/ACM Certification preferred, or willingness to obtain within one year of hire. Exceptional communication, leadership, and organizational skills.
