At The Gardens at Osage Terrace, you will be supported, appreciated, and recognized for the work you do and for the person you are. Responsibilities: Provide direct patient care in an Assisted Living Facility Level 2 Administer medications and treatments as prescribed by physicians. Monitor and assess patient conditions, reporting any changes to the healthcare team. Assist with patient admissions, discharges, and transfers. Document patient information accurately in charts Collaborate with healthcare professionals to ensure quality patient care and outcomes. Maintain and uphold patient confidentiality and adhere to HIPAA regulations. Participate in ongoing training and professional development opportunities. Supervise and support nursing staff, ensuring adherence to best practices and protocols. Address patient and family concerns, providing information and support as needed. Qualifications: Current and valid LPN certification (required). Proficiency in general patient care. Ability to work effectively with diverse patient populations, particularly in geriatrics. Ability to work 12 hour shifts, Holidays and on call Willingness to work a 3x12 hour shift schedule with Weekend On-Call
