We need an Admissions/MDS Nurse to join our team at Coldspring Nursing Care and Rehabilitation in Cold Spring, KY . $29.50 to $44 Per Hour You’ll assess new and existing patients to ensure that all of their care needs are addressed and documented on the MDS. You’ll Be Responsible For: Overseeing the MDS process following the RAI manual Working with other MDS nurses to complete initial assessments for new admissions Properly managing patient admissions scheduling, MDS assessment & care planning Completing required submissions weekly or as directed by the DON Reviewing validation report to ensure error-free submission Obtaining & reviewing QM’s Working with IDT to improve resident outcomes Attending IDT Medicare & Medicaid meetings in collaboration with IDT to complete the MDS process Addressing the needs of residents by developing & updating comprehensive & nurse aide care plans Ensuring non-nursing sections of MDS & CAA’s accurately reflect resident needs Monitoring care plans to identify resident needs Reviewing order recap reports & nurses’ notes to ensure care plans are followed & determine whether additional OBRA MDS assessments are needed Monitoring residents’ change of status & determining need for change per RAI Ensuring accuracy of supportive documentation according to MDS/RAI regulations Working with facility consultants to implement recommended changes Completing AIMS, Fall Risk scale, Braden, Progress note, Care Plan, nurse aide care plan (Kardex) paperwork You’ll Need To Have: A current RN license in the state of Kentucky A flexible work schedule & the ability to work independently An understanding of the MDS process & a willingness to continually learn Knowledge in PDPM & RUG We’re Proud To Offer You: Paid Time Off Paid Training Health Insurance Dental Insurance Vision Insurance Life Insurance Disability Insurance 401K with Employer Match Flexible Schedule Education Scholarships Continuing Education Credits Employee Discounts Exceptional Care & Quality Service are the hallmarks of Coldspring's in-house team of talented and experienced licensed therapists, nurses and caregivers .
