Job Summary: The Addiction Treatment Case Manager provides comprehensive support and coordination of care throughout the addiction recovery process. They assess client needs, connect clients to appropriate resources such as housing, employment, and medical care, and monitor progress toward recovery goals. The Case Manager also collaborates with the treatment team, advocates for clients' needs, facilitates communication between clients and external services, and ensures continuity of care, helping clients overcome obstacles to maintain long-term sobriety. Strong communication, organization, and empathy are essential to effectively support individuals through recovery. Job Duties: Coordinate wellness checks, doctor appointments, and other necessary services to ensure continuity of care for clients. Participate in admissions processes, including intake assessments and orientation, ensuring clients feel supported from the start. Assist clients through the discharge process, providing resources and support to facilitate a smooth transition to independent living. Document critical steps and progress within the recovery program, maintaining accurate and up-to-date records. Address the day-to-day needs of clients, offering support and ensuring their well-being throughout their recovery journey. Be sensitive, respectful, and personable when interacting with clients, building a compassionate and supportive environment. Empower clients by encouraging self-reliance and helping them develop life skills to achieve self-sufficiency. Collaborate with the treatment team to regularly assess clients' progress, make adjustments to individualized plans, and provide ongoing support. Qualifications: Qualified Behavioral Health Technician (meets one of the following): Master's degree or bachelor's degree in a field related to behavioral health; Has a bachelor's degree and at least one year of full time behavioral health work experience; Has as associate's degree and at least two years of full time behavioral health work experience; Has a high school diploma or high school equivalency diploma and a combination of education in a field related to behavioral health and full time behavioral health work experience totaling at least two years; Is licensed a practical nurse, according to A.R.S Title 32, Chapter 15, with at least three years of full time behavioral health work experience; Or has a high school diploma or high school equivalency diploma at least four years of full time behavioral health work experience. Proficient in basic computer skills. Strong time management, organizational, and critical thinking skills to prioritize client needs. Takes responsibility for professional growth and seeks opportunities to improve skills. Communicates effectively through verbal, non-verbal, and written means. Adapts to changing work demands and environments with flexibility. Demonstrates sound judgment and critical thinking in monitoring and responding to client needs.
