Perform resident care, such as medication, treatment, therapy, admissions/discharge assessments, and education of resident and family. 2.Lead the assessment, planning, implementation, evaluation, and documentation of resident condition and care. a.Document and report change in condition, interventions, and resident response. Initiate/follow-through/discontinue acute care plans, as appropriate. b.Up-date Care Plan and problem list as appropriate. c.Communicate Care Plan changes with Nursing Care Coordinator. d.Complete MDS's as assigned. e.Attends care conferences as appropriate. f.Follow-up on Pharmacy on Pharmacy Medication action plans as assigned. 3.Communicate changes in condition to MD, family and team members in a timely manner. 4.Deliver nursing care in compliance with HSS 132 standards and all Federal tags and policies. 5.Collaborate with nursing team, medical consultants, and other team members to ensure resident and family needs are met. 6.Complete double & triple checks of new physician orders. Complete MAR monthly check of orders as delegated. 7.Serve as a consultant to the LPN/Med Tech. 8.Demonstrate effective time management. 9. Complete Daily Medicare assessments as assigned 10. Document in a timely manner in the EMAR and complete all other assigned assessments per facility policy 11. Communicate all patient care needs to MD to request orders for changes of treatment plan Team Leading 1.Give shift to shift report. 2.Inform Nurse Supervisor or DON of staff concerns/issues. 3.Carry out compliance rounds as needed. 4.Facilitate development of knowledge and skills of CNAs and other staff nurses. 5.Perform Relief Supervisor and team leader roles as needed. Communicate to CNAs their job performance. 6.Role model resident care. 7.Advise and assist LPNs, Med Techs, CNAs, volunteers, and CNA Helpers. 8.Assist in interviewing qualified candidates to meet unit needs. 9.Delegate as appropriate. Budgeting 1.Evaluate and report to Supervisor/DON/NHA efficacy of staffing pattern. 2.Use cost effective techniques. Family Support 1.Ensure families of residents are knowledgeable of resident status and prognosis. 2.Inform Nurse Manager(changed from Program Director)/Supervisor of family concerns/issues. Education 1.In coordination with Human Resources Staff Development, provide inservice to other staff members. 2.Advise Nurse Manager/Supervisor/Staff Development Coordinator of topics of interest for continuing education. 3.Identify personal professional strengths and areas for development. 4.Attend continuing education offerings to maintain current knowledge of trends and practices in nursing and long term care. Requirements 1.Registered Nurse licensed in the State of Wisconsin. 2.Minimum 2 year geriatric nursing experience and supervisory experience. 3.CPR certified preferred. 4.Knowledgeable in geriatric care Knowledgeable of HSS 132 and all Federal tags, preferred. 5.Knowledge of Medicare requirements. 6.Ability to work in a partnership environment, which fosters effective team work in meeting the mission of Oakwood. 7.Ability to use independent judgment and make sound decisions. 8.Compliance with Oakwood's Nursing dress code policy. Hours for this position include every other weekend.
