Description:

POSITION/GOALUnder the direct supervision of the Program Director, the Case Manager is responsible for the delivery of services to participants placed in the Post Arrest Diversion Program. Duties include intake and exit paperwork, case planning, crisis intervention and referrals. The job requires a high degree of flexibility in responding to emergency needs of participants as well as good organizational and documentation skills.QUALIFICATIONS• Bachelor’s Degree in Education or related field and/or a minimum of 3 years experience counseling youth and/or young adults• Self-starter, knowledge of adversities facing at-risk youth with special emphasis on girls• Positive attitude towards girls suffering from emotional, physical and sexual abuse• Excellent communication and writing skills and proficiency in major computer software programs• Computer proficiency that includes ability to accurately input data into client informational and case management software and/or hard file• No criminal record and must be able to pass local FDLE background checkRESPONSIBILITIES1. Implement services and activities according to the approved Scope of Services and Evidenced Based Curriculum2. Oversee program enrollment & social service needs assessment; including sanctions by the Department of Juvenile Services and other applicable funding sources3. Establish an individualized care plan for each program participant4. Make referrals of participants and/or family to appropriate agency (mental health, medical, social service, educational, etc.) as needed in conjunction with agency partners5. Oversee, monitor and document monthly progress reports on participants including academic progress, attendance, community service hours, mental health services, and outside agency referrals (community partners)6. Provide monthly progress notes to DJJ and JAC probation officers and other judicial officials as required.7. Facilitate parent participation in monthly Parent Support Group Meetings and/or other program activities as directed by Program Director8. Attend JAC/CBO meetings the second week of every month to maintain the relationship between JPO’s and the agency9. Coordinate recognition activities for participants who complete program requirements10. Comply with the agency’s policies and procedures including confidentiality and equal opportunities11. Produce monthly and/or quarterly reports as required12. Assist with and attend student field trips13. Other duties as assigned Hours Monday through Thursday, 1:00 to 6:30 pmSome evenings and weekends requiredSummer Hours 12:00pm-5:00pmSalary$15 per hour
