Bilingual Case Manager (2 positions)

Description:
The case manager will conduct needs, acuity and resource assessments, and develop long-term care plans with clients, calling on all available resources to meet client needs and dissolve barriers to care. Conduct new client intakes as required. Develop a comprehensive care plan based on the assessment of eligibility, needs, barriers, acuity and established resources. Conduct home-based assessments and other off-site visits as appropriate and in accordance with Agency policies and procedures. Maintain regular contact with clients based on acuity level. Requirements: A Bachelor''s degree in social work, health education, nursing or other social services discipline is required with a demonstrated sensitivity to persons with HIV/AIDS and a working knowledge of medical/psychosocial resources, and the medical and psychosocial complexities of HIV/AIDS. Individuals must have the ability to effectively communicate in verbal and written formats, and collaborate with community service providers, establish effective working relationships with clients. Candidate must be bi-lingual with fluent English/Spanish speaking and writing skills. Individual must have dependable transportation and current Texas Driver''s License and active auto liability insurance.
