The Mission Neighborhood Health Center (MNHC) is a non-profit, multi-site community health organization offering comprehensive health services in the Mission and Excelsior Districts of San Francisco. The center offers primary health care services, adult medicine, pediatrics, family planning, OB/GYN, HIV and homeless services. We advocate for health equity and deliver innovative, high-quality services responsive to the neighborhoods and diverse communities we serve with a focus on the Latino Spanish-Speaking Communities.We are currently looking for qualified candidates for our Health Enrollment Navigator role. Please note: this role requires an applicant to be bilingual (English/Spanish) . This position reports to the Director of Patient Services and is a Regular, Full Time, Non-Exempt role with a starting hourly range of $20.00 to $24.00 with full benefits . Primary Objective:The Navigators Program provides a variety of outreach and navigation services to various vulnerable populations who are potentially eligible for enrolling or re-enrolling in Medi-Cal (California’s state-run and funded Medicaid program), which provides health coverage to families with very low income, as well as expectant mothers, persons with a range of disabilities, and immigrants and families with mixed immigration status, et al. Essential Functions & Responsibilities Engagement Works with staff to develop and implement a plan for community engagement that will connect residents to resources through the scheduling and follow-up of appointments . Leads public education activities at all four clinic sites that increases knowledge of health programs, the benefits of health coverage, and the eligibility requirements. Develops language accessible outreach materials in Spanish and English that have information about Medi-Cal services, office location, office hours, and contact information. Collaborates with community-based organizations to inform their stakeholders of healthcare opportunities, assists them with the application process, and in establishing a health home. Coordinates workshops, clinics, and community meetings, including supporting residents and supporting the needs of co-workers, and partner staff. Conducts weekly outreach and education at local schools, special events, and heavy use corridors, i.e., Mission Street, Valencia, Geneva Ave, and other neighborhoods where the Latinx community gathers. Coordinates with the other Health Education Navigators by meeting regularly and adjusting and enhancing the team’s work to maximize outreach and engagement. Participates in regular team meetings to evaluate and develop strategies to address ongoing and emerging related issues in the district. Compiles, develops, and maintains file notes, statistical reports, and participant records to better understand our internal success rate and trends, and for grant reporting. Maintains the highest standards of professional conduct in relation to information that is confidential in nature. Pays strong attention to detail and manages competing priorities effectively, and participates in nurturing a harmonious and teamwork-oriented work environment New Enrollment and Renewals Contacts patients whose Medi-Cal has lapsed and assists them with renewing their membership. Identifies residents who qualify for Medi-Cal enrollment and assists them with applying. Data Management and Privacy Protection Protects sensitive patient health information from being disclosed without the patient's consent or knowledge by adhering to the standards of the Health Insurance Portability and Accountability Act of 1996 (HIPAA). Other Duties Responds to other duties as assigned by the supervisor. Qualifications Bilingual in English and Spanish is required. High School diploma or equivalent. Associates Degree preferred. Excellent verbal and written communication skills. Ability to work in an independent, calm and reassuring manner using good boundaries and good judgment; to be reliable, dependable, and flexible. Ability to work within all the rules, regulations, and policies that govern the Health Center’s compliance. Organizational skills required as well as a flexible schedule and hours may vary and include evening work. Experience in counseling working-class Latinx immigrants, and or providing community social services to underserved communities. A commitment to social and economic justice, and knowledge of issues impacting working class immigrants in San Francisco. Experience completing government funded health care program applications. Strong computer skills including intermediate MS Office skills, Internet research and experience with database systems.
