As a Critical Care Coordinator, you play a crucial role in the Care Coordination and Support program. Your responsibilities include evaluating and interpreting referral information, conducting strength needs assessments, and creating individualized service plans. You'll facilitate Crisis Prevention and Crisis Safety plans, engage with families regularly, and ensure that youth's needs are met through collaboration with various stakeholders. We're looking for someone who can work a full-time schedule while meeting the following qualifications: Bachelor's degree in counseling and guidance, rehabilitation counseling, social work, education, psychology, or a related human service field 2 years of supervising experience Knowledge of community programs, services, evidence-based practices, and various treatment modalities used to work with youth and their families to navigate child-serving systems Ability to actively participate in a fast-paced, multidisciplinary team environment Valid driver's license and proof of vehicle insurance Strong organizational and time management abilities Excellent interpersonal and written communication skills Ready to make a difference?
