The Case Manager - Intensive manages a case load of adults with severe and persistent mental illness through the activities of planning, linking, monitoring, advocacy, and assessment. MINIMUM EDUCATION & EXPERIENCE BA/BS in Social Sciences (Social work, psychology, sociology, etc.) from an accredited college or university1 year of experience working in mental health or related fieldValid FL driver's license PREFERRED EDUCATION & EXPERIENCE MA/MS in Social Sciences (Social work, psychology, sociology, etc.) from an accredited college or university2 years of case management experienceCase Manager Certification (CBHCM) - FloridaKNOWLEDGE & SKILLSFamiliarity with 65E regulations, Baker Act, and Marchman ActAbility to interpret and abide by confidentiality laws, abuse reporting guidelines, and client rights requirementsProven track record of abiding by a code of ethical and practice standardsBasic counseling skillsCase management skillsAdvanced communication skills, both written and verbalExcellent organizational and time management skillsGeneral typing and computer literacy skillsProficiency in educating adults on illness, medication compliance, community resources, money management, and basic life skillsExperience with assessment and treatment planningCapability to handle verbally or physically aggressive clientsKnowledge of psychiatric medication and community resources RESPONSIBILITIES CLIENT CAREConduct Assessment Interviews with clientsAccompany clients to and link them with community resources, as neededCounsel clients and family members, where appropriate, on relevant clinical issuesPerform weekly home visits to all clients on case loadCLIENT ADVOCACYAdvocates for client, such as assisting with completing applications for services and discussing clients needs with providersCoordinates care with psychiatrists, therapists, medication clinics as well as with external services such as hospitals, social service agencies, and housing agenciesMonitors treatment process to ensure needed services are provided on a timely basis RECORD KEEPING & OTHER ADMINISTRATIONAttend staff meetingsDocumentation, including progress notes, progress reviews, and other reportsDevelop comprehensive service plansCompose and type letters, correspondence, etc.Complete staff activity reports, PB2 and functional assessment rating scalesAccomplishes locus assessment at admission and every six months, as needed to determine level of careAchieve daily entries into credible (EMR) schedulePowered by JazzHR
