The Hospital Liaison works in partnership and collaboration with interdisciplinary team members at the hospital with the goal of providing quality patient care and enhanced communication between hospital and clinic operations. Essential Functions: Responsible for providing the hospital with the clerical support necessary to ensure a smoothly functioning workflow and connection to the hospital from MPHC. Work directly with Medical Providers and Hospital team to provide support services to assist with the timely discharge/transfer of patients from the hospital to MPHC. Manages telephone messages from their assigned to pods daily. Obtain Medical Records from eCW for patients that are present at the hospital. Manages and track patient cases that are no show or cancel an appointment and scheduled them in the appropriate timeframe per provider recommendation. Schedule newborn visits and other follow up appointments with other departments as needed. Manages eCW Jellybeans for their assigned Pod daily. This includes Labs/DI's, TE's, Documents, actions and consult pending. Diligent in record keeping by keeping an ongoing file system indicating patient names, addresses, due dates, appointment dates, risk factors, delivery information, and other relevant information. Initiates or writes letters or electronic communication with patients who are unavailable via telephone. Maintains accurate and complete documentation in the patient records to continuously improve the patient services process and meet regulatory requirements. Schedule C-section and inductions, as well as registering newborns into the electronic health record. Utilize and/or schedule interpreter services as needed by the patient. Must be able to embrace differences among people and able to interact with internal staff as well as external contacts in a culturally competent and respectful manner. Performs other duties as assigned. QUALIFICATIONS Minimum Qualifications: * Bachelor's degree or 2+ years related experience and/or training.
