Full-Time Hospice Home Health CNA/HHA Dependable Health Services is seeking a compassionate and dedicated Full-Time Hospice Home Health Certified Nursing Assistant (CNA) or Home Health Aide (HHA) to provide outstanding care and support to our terminally ill patients. In this role, you will work closely with a multidisciplinary team of healthcare professionals and provide vital assistance to patients in the comfort of their own homes, ensuring their physical and emotional well-being. Responsibilities: Provide basic patient care under the supervision of a registered nurse or licensed healthcare professional. Assist patients with activities of daily living, such as bathing, dressing, grooming, and feeding. Administer medication following the prescribed schedule set by the healthcare team. Monitor vital signs and report any significant changes or concerns to the appropriate healthcare professional. Assist with mobility and transfer of patients, considering their comfort and safety. Help maintain a clean and safe environment for patients by ensuring proper infection control measures and organizing the patient's living space as needed. Offer emotional support and companionship to patients and their families, demonstrating empathy and respect during difficult times. Provide necessary comforts and personal care to enhance the patient's quality of life. Collaborate with the hospice team to develop and implement personalized care plans for each patient, taking into consideration their individual needs and preferences. Document all care provided accurately and timely in the patient's medical record, ensuring compliance with regulatory standards and ethical guidelines. Participate in regular team meetings and training sessions to enhance knowledge and skills in hospice care. Attend to other duties as assigned by the supervisor or hospice management. Qualifications: High school diploma or equivalent. Certification as a Nursing Assistant or Home Health Aide (CNA/HHA) from an accredited program. Previous experience in hospice, home health, or long-term care settings is preferred. Current CPR certification. Strong communication and interpersonal skills to effectively communicate with patients, families, and the healthcare team. Ability to provide physical and emotional support to patients and their families during difficult times. Compassionate, empathetic, and patient-centered approach towards end-of-life care. Exceptional organizational and time management skills to handle multiple tasks efficiently. Ability to work independently, demonstrate adaptability, and maintain a positive attitude. Knowledge of infection control practices and ability to follow procedures to maintain a safe environment. Proficiency in using electronic medical record systems is preferred. This is a full-time position that may require flexibility in working hours, including weekends and holidays, to ensure continuous care and support for our patients.
