Korzen Health is working with the partnership of Healogics to find a Post Acute Nurse Practitioner. We are looking to add an experienced NP to cover local SNFs 2/3 days per week. Procedure experience within the last 2 years IS required. For example: sutures, central lines, intubations, and anything that would be very specialized. This is a part time, 2/3 days/week position - flexible hours. Post-Acute Care wound care program located in Rockford, IL market. This is 100% wound care position. 52 hours of paid training included.This is a mobile position. We have 2/3 days of work in Rockford, IL market area. You will be traveling to skilled nursing facilities (SNFs) to provide wound care to patients. The SNF locations are DeKalb, Freeport, Rockford, Oregon, Lena, Prairie Crossing, Franklyn Grove. Scope of Practice Includes: You will average around 18-20 patients a day. Performing basic wound care (dressing changes, chemical debridement) with some minor procedures. This will be a part-time 2/3 day a week job. No Nights, No Call, No weekends. Flexible. Compensation Model is salary with RVU after 1 year. You will work with Adult and Geriatric patients.
