Summary This social worker would serve to provide psychosocial support, care management, and care coordination for the neurorehabilitation patient population across the three adult outpatient neurorehabilitation sites - BW04 MedRehab, Brighton Center for Specialty Care, and Canton Health Center. Additionally, this role will provide crisis intervention, safety planning, case management services and community referrals, as well as patient education, group support, and facilitation of multi-disciplinary team meetings. Responsibilities* You will be responsible for: Bio-psychosocial assessments Evaluation of psychosocial needs and provision of billable social work interventions with a high degree of autonomy Care Management and care coordination, scheduling coordination Navigation of barriers to care Transition support/care continuity Facilitating patient and family participation in appropriate health care decision making Crisis intervention Patient and/or family support and counseling Support group co-facilitation Liaison and referral to community agencies Participation and/or facilitation of multi-disciplinary team meetings, rounds, educational activities, and patient advisory board Referral monitoring and management Working directly with patients both face-to-face and virtually by phone or video visit. Required Qualifications* A Master's degree in Social Work from a school accredited by the Council of Social Work Education A current license to practice social work in the State of Michigan Desired Qualifications* Previous experience with medical and mental health social work. Skilled in developing rapport and effectively communicating with a diverse patient population. Efficient, accurate assessment of patient needs and effective case dispositions. Effective organizational skills, flexibility, self-motivation, the ability to effectively prioritize multiple competing tasks in a dynamic medical clinic. Independent, sound decision-making is necessary for success in this position. Effective collaboration with a large multi-disciplinary team. Proficiency in keeping accurate and timely clinical records by computer. Work Schedule This is full time, 40 hour/week position. The typical schedule is Monday-Friday with start and end times 8:00-4:30 or 8:30-5:00. Work Locations This position is hybrid with work that can be done remotely and requiring periodic onsite presence at the following three locations. Briarwood will be the primary site requiring weekly onsite presence. Briarwood Building 4-Med Rehab, 355 Briarwood Cir. Bldg 4, Ann Arbor, MI 48108 Brighton Center for Specialty Care, 7500 Challis Rd, Brighton, MI 48116 Canton Health Center, 1051 N Canton Center Rd, Canton, MI 48187 Underfill Statement This position may be underfilled at a lower classification depending on the qualifications of the selected candidate. Advancement through the career ladder at the Expert or Clinical Scholar Level is required for placement in the MSW Senior Classification. Background Screening Michigan Medicine conducts background screening and pre-employment drug testing on job candidates upon acceptance of a contingent job offer and may use a third party administrator to conduct background screenings. Background screenings are performed in compliance with the Fair Credit Report Act. Pre-employment drug testing applies to all selected candidates, including new or additional faculty and staff appointments, as well as transfers from other U-M campuses.
