LINE OF RESPONSIBILITY: The Nurse Outreach Worker reports to the Grants Manager/Special Projects Coordinator regarding administrative matters. They report to the Nursing Supervisor regarding clinical matters. PURPOSE: To provide outreach services to area elders regarding health care and support services; to provide nursing services; and to keep program statistics and write reports. RESPONSIBILITIES: Schedule and conduct outreach visits to area elders. Check patients' vital signs and perform other nursing functions, as needed. Conduct home safety assessments and arrange for needed repairs, as appropriate. Assess the need for durable medical equipment and assist with acquisition, as needed. Educate patients about the need for Health Care proxies and assist with their completion, as needed. Educate family members and significant others about how to care for loved ones at home. Educate patients about the need for health care appointments with physicians and specialists and assist them to make and keep appointments, as necessary. Arrange transportation for patients to appointments and events, as appropriate. Provide information and guidance about community resources, health care services, community activities, and alternative living arrangements (nursing homes, rest homes, retirement homes, etc.) as appropriate. Draw blood per physician's orders. Refer patients for more extensive services, e.g. VNA, Physical Therapy, Highland Valley Elder Services, etc. Arrange for a medical response system for elder homes. Assess medications, assist with medication compliance, and assist with completion of the File of Life for patients, as needed. Assist patients to obtain handicapped parking placards. Act as program liaison to Council on Aging, community organizations, and individuals and represent the program and the agency in a professional manner. Update and distribute program flyers and information, as needed. Conduct annual flu clinics and monthly blood pressure clinics. Address program inquiries and patient phone calls. Compile regular program statistics and patient demographics and write related reports. Attend related meetings. Other related duties as assigned by supervisor. This position requires some travel within the community. QUALIFICATIONS: Must be a Licensed Practical Nurse or Registered Nurse with a valid Massachusetts license. A valid Massachusetts driver's license is also required. Must have a minimum of two years of related experience working with elders. Good organization skills and the ability to relate respectfully and courteously to others are required. Must be able to work independently. Reliable transportation is required, this position requires some travel within the community. COVID Safety Measures: All employees are required to be fully vaccinated against COVID-19, including the first booster dose. All staff are required to wear masks while on-site, unless in an isolated space or actively eating or drinking. All staff must complete a health attestation before entering any HCHC facility.
