JOB SUMMARY: Performs a wide range of clerical tasks and computer file maintenance to support record processing functions. Timely responds to internal and external customer requests for health information. Retrieves, maintains and files medical records and related patient information. Understands and utilizes basic risk management guidelines regarding release of patient information. This position will provide clerical functions and specific patient care duties to facilitate efficient and comprehensive services in the clinic. JOB DUTIES: 1. Retrieves and distributes patient medical records and documents for scheduled and unscheduled patient care as requested; re-files and/or archive scans records and other documents both on-site and off-site.2. Responds to telephone, customers and computer-generated requests and prioritizes requests in a prompt manner.3. Accurately and efficiently analyzes clinic and hospital medical records including, but not limited to, prepping, scanning, indexing, routing and quality analysis.4. Uses problem solving methodology to locate or identify lost records, reports, computer duplication and Admission, Discharge, Transfer (ADT) errors.5. Promptly processes health information documents including birth certificates, death certificates, cremation approvals, living wills/power of attorney, etc., including utilizing outside registry systems. 6. Ensures appropriate routing of external clinical notes to providers.7. Purges medical records according to guidelines and prepares documents for storage and destruction.8. Protects Patient Confidentiality. Maintains the integrity of the EMR and paper medical records. Receives, verifies and processes release of information requests and respects the confidentiality of all information. 9. Picking up, sorting, completion and preparation of forms and distribution of all outgoing clinic mail and faxes.10. Prepare immunization records for patients by reviewing MIIC, EMR notes, and/or chart.11. Sending tests out for interpretations, scanning and routing of the documents and preparing the appropriate charge tickets for coding.12. Faxes information accurately to both internal and external facilities and utilizes the ROI module when necessary.13. Works in collaboration with medical staff and nursing staff on a daily basis for chart completion, order completion and routing of reports.14. Other duties as assigned. QUALIFICATIONS: 1. Must be 18-years of age or older.2. One-year experience in medical record processing is preferred.3. Basic computer skills preferred.4. Must possess a valid MN Driver's License and have a satisfactory driving record as verified through DVS.5. Must possess effective communication, problem-solving and time management skills6. Must have customer service knowledge and skills7. Strong organizational skills.8. Demonstrate participation in & uphold Grand Itasca's Mission, Vision & Values.9.
