$2500 sign-on bonus available Physician / Medical Provider HCHC is a non-profit, federally qualified health center with a mission to create access to high-quality integrated health care and to promote well-being for individuals, families, and our communities. We provide medical, dental, behavioral health, eye care services, and community programs to everyone, regardless of ability to pay. Title of Position: Physician / Medical Provider Schedule: Full-time Monday-Friday Compensation: competitive with $2500 sign on bonus available! FLSA: Exempt Location: Huntington Health Center LINE OF RESPONSIBILITY : The Physician / Medical Provider reports to the Medical Director. The Physician / Medical Provider functions within the parameters and requirements of the Massachusetts Board of Medicine. PURPOSE : To provide comprehensive, quality medical care and coordination services for HCHC patient s . Key Responsibilities: Care of HCHC patients: outpatient setting History/exam/assessment/diagnosis Order and review test results Treatment: prescriptions/devices/modalities Immunizations Referrals Follow-up Review of previous records/hospitalizations/consultations Phone messages (pharmacy, patients, families, agencies, insurance, consultants) Medical back-up to Mental Health and Dental Departments Supervise all aspects of patient care in the medical department as appropriate: Nurse visits Hope nurse Advance practice providers (on-site consultation, reviewing charts, phone back-up to APPs at alternate sites) The Physician will participate in additional activities related to the functioning of the Medical Department, which may include: Community Service Events Administrative work at the health center Guideline/policy development Quality improvement activities Hiring/evaluation of staff Participation in relevant committees In a cooperative fashion, ensure that all health centers are open to serve patients during scheduled hours, including evening, weekend and holiday hours as needed Participate in on-call responsibilities. Delivery of urgent/emergent care to people at the HCHC that are not our patients (e.g. family members). Note : as always, anyone seen emergently at the Health Center is considered a Health Center patient, even though it may be their first encounter. Review of colleague's lab work/prescription refills/urgent phone messages Home visits to patients of HCHC as appropriate. Participate in team care of patients, collaborating with other departments Continue to maintain Massachusetts licensure and participate in related CME activities, as appropriate Maintain current CPR certification Maintain proper patient records including locking notes within 72 hours. Serve as a clinical resource to staff Other duties as determined by the Medical Director or Assistant Medical Director Qualifications (skills & experience): Massachusetts medical license and Board Certification in Internal Medicine, Pediatrics, Family Practice, or double Certification. Must be committed to community health and to serving the Hilltown population, regardless of individuals' ability to pay for services COVID Safety Measures: All employees are required to be fully vaccinated against COVID-19, including the first booster dose. All staff are required to wear masks while on-site, unless in an isolated space or actively eating or drinking. All staff must complete a health attestation before entering any HCHC facility.
