Senior Manager, Community Partners Program Organization Summary: Community Care Cooperative (C3) is a 501(c)(3) non-profit, Accountable Care Organization (ACO) governed by Federally Qualified Health Centers (FQHCs). Our mission is to leverage the collective strengths of FQHCs to improve the health and wellness of the people we serve. We are a fast-growing organization founded in 2016 with 9 health centers and now serving hundreds of thousands of beneficiaries who receive primary care at health centers and independent practices across Massachusetts. We are an innovative organization developing new partnerships and programs to improve the health of members and communities and to strengthen our health center partners. Job Summary: The Senior Manager, Community Partners Program will report to the Senior Director of Behavioral Health and will work closely with the Practice Transformation, Quality and Care Management teams. The Senior Manager will interface with senior management, peers, FQHCs/Practices, MassHealth, MassHealth Behavioral Health vendor and other key stakeholders, and the consumer community. The Senior Manager must be innovative, comfortable with ambiguity, well-organized, and committed to moving quickly and collaboratively. They must also have strong analytical skills and communicate clearly and succinctly in both written and verbal communication. Responsibilities: • Assist in the design and advancement of the Community Partners Program • Work with the Senior Director of Behavioral Health to develop project plans to monitor and track the Community Partners performance • Lead the Community Partners Program from design to implementation • Build relationships between the Community Partners (CPs) and FQHCs/Practices • Develop partnerships such as those with Community Behavioral Health Centers, behavioral health facilities and community organizations and social service agencies to improve care coordination and access to care • Ensure that all project tasks are delivered on time, within scope, and within budget • Use quality improvement methodology as needed to achieve targeted goals & objectives • Work with MassHealth and the provider networks to increase coordination and collaboration with the FQHCs/Practices • Work with the CP vendors to improve workflows related to integrated care, access to treatment, transitions of care, care coordination, HEDIS/NCQA measures, program development and process improvement, etc. • Remain up to date on MassHealth requirements related to the Community Partners program • Closely collaborate, including co-design of clinical programs and workflows, with the Care Management departments • In conjunction with the Senior Director of Behavioral Health and the Senior Director of Care Management, identify and replicate best practices in care coordination • Ability to interpret data and identify areas for improvement in service delivery by the CPs • Perform other duties as assigned Required Skills: • Previous experience working in Community Partners Program or other care coordination program required • At least 5-7 years of experience in providing services to Medicaid/Medicare consumers • Working knowledge of behavioral health • Previous management experience required • Proficient in Microsoft Office, specifically excel with building and maintaining spreadsheets, and Power Point development of slide presentations • Strong project management, analytics and program development experience • A self-starter who can exercise high degree of initiative, judgement, discretion and decision making to achieve objectives • Works well with others inside the organization and outside, understanding different perspectives and finding collaborative ways to the get the job done • Comfortable multi-tasking and working at a fast pace • Behaves with great integrity and produces accurate work with close attention to detail, especially in the completion of final deliverables to internal and external stakeholders • Experience with anti-racism activities, and/or lived experience with racism is highly preferred • Must be able to remain in a stationary position 50-75% of the time Desired Other Skills: • Familiarity with the MassHealth ACO program • Familiarity with Federally Qualified Health Centers • Experience with anti-racism activities, and/or lived experience with racism is highly preferred Qualifications: • Master's Degree in Social Work, Public Health or related field required • LICSW/LMHC preferred ** In compliance with Covid-19 Infection Control practices per Mass.gov recommendations, we require all employees to be vaccinated consistent with applicable law. ** Job Posted by ApplicantPro
