Work Location: Bradenton, FL As part of this role, you will: Conducts care coordination activities as defined in the Healthy Start Program Standards and Guidelines published by the State of Florida which include: Conducts assessments of participants and other family members' needs and support systems, both in the participant's home and in the office, utilizing program assessment forms Provides ongoing case monitoring of participants' condition and continually updates linkages with appropriate service providers to ensure continuity of care and increased participant quality of life Maintains a working knowledge of community resources for at-risk pregnant women Maintains a working knowledge of community resources for at-risk pregnant women, infants, young children, and their families and understands procedures for accessing them Attends community meetings and workshops as required, and participates in required Healthy Start activities, trainings, and meetings What you need to bring to this role: Bachelor's Degree in a health or social services field and at least two years of experience in a related field is required Microsoft Office Products - Word, Excel, Outlook, PowerPoint proficiency required 2+ years' experience in maternal and child health care preferred 1+ years' experience in Healthy Start program beneficial Valid driver license required with the ability to move between sites as needed Knowledge of local, state and national health and human service resources Bilingual in English and Spanish or Creole preferred, but not required Excellent oral, written, and communication skills Excellent time management and organizational skills **MCR Health is a drug free workplace.
