Conduct comprehensive member assessments that include medical, functional, psychosocial, and behavioral health needs. Utilize interviewing techniques and active listening to obtain member information. Identify member needs. Work with members to develop health care goals and identify possible barriers. Identify relevant community resources available based on member needs. Refer members to appropriate programs and services. Work collaboratively with community providers to meet member needs. Follow up with members to monitor progress towards plan goals making changes or recommendations as necessary. Educate members on preventative healthcare measures; plan benefits available; signs and symptoms of worsening conditions; and appropriate utilization of services based on need. Present member case information to interdisciplinary care team and obtain input into plan of care as necessary. Research member-related health information using internal and external systems. Document information in department computer system. If you reside within a commutable distance of Milwaukee, WI, you will have the flexibility to work remotely* as you take on some tough challenges.
