As part of a Care Management team who will manage complex members, the Field Case Manager will be the primary care manager for a panel of members with complex behavioral health needs (i.e., mental health or substance use disorders). Additionally, this position will provide behavioral health support to the broader team. Care coordination activities will focus on supporting member's medical, behavioral, and socioeconomic needs to promote appropriate utilization of services and improved quality of care.In this Field Case Manager role, will be an essential element of an Integrated Care Model by relaying the pertinent information about the member needs and advocating for the best possible care available, and ensuring they have the right services to meet their needs.If you are located in Miami, Florida, you will have the flexibility to work remotely* as you take on some tough challenges. Primary Responsibilities: Assess, plan, and implement care strategies that are individualized by patient and directed toward the most appropriate, lease restrictive level of careIdentify and initiate referrals for social service programs, including financial, psychosocial, community and state supportive servicesManage the care plan throughout the continuum of care as a single point of contactCommunicate with all stakeholders the required health-related information to ensure quality coordinated care and services are provided expeditiously to all membersAdvocate for patients and families as needed to ensure the patient's needs and choices are fully represented and supported by the health care team Expect to spend about 80% of your time in the field visiting our members in their homes or in long-term care facilities. You'll need to be flexible, adaptable and, above all, patient in all types of situations. You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in. Required Qualifications: Four-year degree plus 2+ years of relevant experienceNo degree, 6+ years of relevant social service or case management experience1+ years of experience with MS Office, including Word, Excel, and OutlookExperience in long-term care, home health, hospice, public health, or assisted livingExperience with local behavioral health providers and community support organizations addressing SDOH (e.g., food banks, non-emergent transportation, utility assistance, housing/rapid re-housing assistance, etc.)Bilingual SpanishAccess to reliable transportation and the ability to travel within assigned territory to meet with members and providers Preferred Qualifications: LTC Case management experienceExperience with electronic charting Experience with arranging community resources Field-based work experienceExperience in serving individuals with co-occurring disorders (both mental health and substance use disorders)Background in managing populations with complex medical or behavioral needs If the hired individual resides in Florida (office based or telecommuting) this position requires the AHCA Level II background check (fingerprinting) by the State of Florida for all clinicians that have direct face to face contact with members OR employees who will have access to confidential patient data and will require renewal every five years. *All employees working remotely will be required to adhere to UnitedHealth Group's Telecommuter Policy At UnitedHealth Group, our mission is to help people live healthier lives and make the health system work better for everyone.
