As a Transplant Case Manager, you will drive positive outcomes by supporting a diverse member population with education, advocacy and the connections to the resources they need. This role is required to work Monday - Friday from 8:00am to 5:00pm (with some flexibility). You'll enjoy the flexibility to work remotely * from anywhere within the U.S. as you take on some tough challenges. Primary Responsibilities: Makes outbound calls to assess members' current health statusCase manages members in the pre-operative and post-operative phasesIdentifies gaps or barriers in treatment plansProvides patient education to assist with self-managementInteracts with Medical Directors on challenging casesCoordinates care for membersMakes referrals to outside sourcesCoordinates services as needed (home health, DME, etc.)Educates members on disease processesEncourages members to make healthy lifestyle changesDocuments and tracks findingsMakes welcome home calls to ensure that discharged member receive the necessary services and resources You'll be rewarded and recognized for your performance in an environment that will challenge you and give you clear direction on what it takes to succeed in your role as well as provide development for other roles you may be interested in.Years of post-high school education can be substituted/is equivalent to years of experienceRequired Qualifications: Current, unrestricted RN license in your state of residenceMultiple state licensure (in addition to Compact License if applicable) or ability to obtain multiple state nursing licenses3+ years of experience as a Registered Nurse (RN) 1+ years of Transplant experience working with solid organ or bone marrow as a NurseBasic level of proficiency with Microsoft Word, with the ability to navigate a Windows environmentAccess to high-speed internet from home (Broadband cable, DSL or fiber)Dedicated workspace from home Preferred Qualifications: Certified Case Manager (CCM)5+ years of Transplant experience working as an RN3+ years of experience working as a Transplant CoordinatorManaged care experienceCase Management experienceReside in compact RN license state California, Colorado, Connecticut, Nevada, New Jersey, New York, Rhode Island, or Washington Residents Only: The salary range for California, Colorado, Connecticut, Nevada, New Jersey, New York, Rhode Island or Washington residents is $56,300 to $110,400 per year. Pay is based on several factors including but not limited to education, work experience, certifications, etc. In addition to your salary, UnitedHealth Group offers benefits such as, a comprehensive benefits package, incentive and recognition programs, equity stock purchase and 401k contribution (all benefits are subject to eligibility requirements).
