Qualifications for Occupational Therapist - Occupational Therapist Home Health : Valid California Occupational Therapist (OT) license CPR Certification Reliable transportation and valid driver’s license with proof of insurance required. Responsibilities and Duties: Complete discharge planning by consulting with physicians, nurses, social workers, and other health care workers. Document patient care services via charting in a timely manner. Promote maximum independence by selecting and constructing therapies according to the individual's physical capacity and interest Evaluate results of occupational therapy by observing, noting, and evaluating patient's progress; recommend and implement adjustments and modifications Implement physician-ordered occupational therapies for self-care activities Help the patient develop or regain physical or mental function, or adjust to their disability This job description should not be interpreted as an exhaustive list of all responsibilities, duties, or skills that may be required of the employee.
