A DAY IN THE LIFE OF A SUBSTANCE ABUSE CASE MANAGER As a Substance Abuse Case Manager with our nonprofit, you're responsible for providing services to clients who have substance use disorders, are in transitional housing, have a severe mental illness, or struggle with other complicated behavioral health issues. You counsel clients on the treatment and rehabilitation process as well as advise them of their rights and responsibilities. As needed, you accompany clients to appointments and help them develop a manageable treatment and work schedule. You also assist clients in collecting documentation for social security and housing applications as well as provide advocacy services throughout the application process. In addition, you provide community-based services in a variety of settings, including the client's home, treatment facilities, transitional housing sites, or shelters. Services are sometimes administered outside of traditional working hours to promote stabilization, income security, and program engagement. You provide intake and orientation services to help clients understand and explore their needs, strengths, interests, aptitudes, experiences, capabilities, career goals, and family situations. Using a client-centered model, you develop and maintain comprehensive service plans that cater to the unique needs of clients with untreated or unstable substance use, mental health issues, or medical conditions. You address support service needs for your clients and make appropriate referrals to training or other resources for areas such as housing, legal services, medical conditions, domestic violence, transportation, childcare, and employment services. Based on substance use assessments, you also make referrals to substance use treatment or harm reduction services. You maintain ongoing communication with clients to monitor their progress, identify early warning signs of crisis or relapse, and keep them engaged with our program. Using your psychology expertise, you also provide vocational counseling, intervention, and program action planning services, such as advocating for clients at treatment programs and work sites. You capture and record employment information and monitor newly hired clients for their first 6 months on the job. Your case management work can sometimes be challenging, but there's nothing else you'd rather do. QUALIFICATIONS FOR A SUBSTANCE ABUSE CASE MANAGER Bachelor's degree in social work or a related field such as psychology, vocational rehabilitation, or occupational rehabilitation from an accredited institution Credentialed Alcoholism and Substance Abuse Counselor (CASAC A+) 2+ years of case management experience OR a combination of training and work experience deemed acceptable by the agency Covid-19 vaccination Bilingual would be a plus, but multiple factors will be taken into consideration.
