General Summary: Under the direction of the Supervisor, Case Management Services, the Medical Case Manager provides culturally competent and trauma informed services to all consumers, assists consumers to design and implement strategies for obtaining services and supports that are goal-oriented and individualized. A Medical Case Manager shall be responsible for assessment, planning, linkage, advocacy, coordination and monitoring developed through the person-centered planning process. The individual in this position has overall responsibility for ensuring that care is provided at the appropriate level of care based on medical necessity and to assess the patient for transition needs to promote timely throughput, safe discharge and prevent avoidable readmissions. Care Coordination by demonstrating throughput efficiency while assuring care is the right sequence and at appropriate level of care. The Medical Case Manager incorporates LifeWays' mission, vision, and values into all decision-making processes. Essential Functions: Welcomes all individuals by demonstrating empathy and providing opportunity for the person presenting to describe their situation, problems and functioning difficulties, exhibiting excellent customer service skills, and working with them in a non-judgmental way. Collects information from individuals for decision-making and reporting purposes. Performs all duties as assigned to ensure the CSM department runs efficiently and effectively. Duties include but are not limited to assessments, treatment planning, referrals, care coordination, formal reviews, discharge/transitions. Coordinates all referrals from provider network to Meds-Only program. Monitors Meds-Only caseload and completes updated assessments and treatment plans as needed to continue appropriateness of continued services. Coordination of care within the provider network when referring for additional services due to Level of Care need changes. Coordination between psychiatrists, primary care physicians and other community providers when necessary to discharge from services to ensure appropriate after care planning and follow-up. Provides Medical Case Management to individuals through in person and telephonic communications with the patient, physician, Psychiatrist other health care providers, and others. Assures that the plan of service identifies what services and supports will be provided, who will provide them, and how the case manager will monitor (i.e., interval of face-to-face contacts) the services and supports identified under each goal and objective. Performs transition planning services and assessment / reassessment as permitted and upon due date. Ensures accurate medical necessity screening and submission for Psychiatry Review Ensures implementation or oversight of the transition plan Oversees implementation of the individual plan of service, including supporting the consumer's dreams, goals, and desires for optimizing independence; promoting recovery; and assisting in the development and maintenance of natural supports. Utilizes their medical knowledge to discuss the current treatment plan with the psychiatrist and discuss alternate treatment plans. Assures the participation of the consumer on an ongoing basis in discussions of his plans, goals, and status. Coordinates the consumer's services and supports all providers, making referrals, and advocating for the consumer. Medical Case Manager will meet with consumers based on service need but at minimum every 6mths can be quarterly. Assures coordination with the consumer's primary and other health care providers to assure continuity of care. Facilitates the transition (e.g., from inpatient to community services, school to work, dependent to independent living) process, including arrangements for follow-up services. Assures the patient is in the appropriate status and level of care based on Medical Necessity. Assists consumers with crisis planning. Identifies the process for after-hours contact. Delivers evidenced-based interventions as trained and appropriate to the consumer needs. Leading and facilitating Complex Case Review Required Education/Certification/Licensure: Case Manager I: Bachelor's degree in a human service field or bachelor's degree in social work, and meets qualifications as QIDP, QMHP or CMHP. Case Manager II: Bachelor's degree required in social work, or human services field, and is Fully Licensed Bachelor's Social Worker (LBSW). Meets qualifications as a QIDP, QMHP, or CMHP. Preferred Education/Certification/Licensure: Bachelor's in Social Work with LLBSW or LBSW with the State of Michigan. Master's in Social Work with LMSW licensure with the State of Michigan. Master's degree in Counseling, Family & Marriage Therapy or Psychology. Certified Advanced Alcohol and Drug Counselor (CAADC)
