Community Nurse The wellness/community nurse RN/LPN is responsible for provision and/or oversight of wellness care and coordinating health services for community residents. The wellness Nurse will also work with the community to develop healthy plans. Responsibilities Communication with resident support services, residents and families, championing quality of life for all residents. Monitors progress through blood pressure readings/vitals, weight monitoring and evaluations. Provides information and educates residents in-person and telephonically regarding healthy lifestyles. Contacts physicians and/or specialists regarding health status and assists arranging appointments. Participates in Wellness Fairs, outreach Events and educational events. Conducts Wellness Assessments Participates in Orientation, in-services and staff-meetings as scheduled. Participates in on-call duties as defined by the on-call policy. Follows privacy policies of Inspired Homecare and maintains the confidentiality of protected health information (PHI) Qualifications: State RN/LPN license required. CPR Certification 1-3 years of nursing experience in a hospital setting or community health setting involving direct nursing care Excellent communication and time management skills Compassionate and friendly demeanor Ability to travel to multiple patients per day. Overnight travel may be required. Preferred knowledge of Home Care Home Base software
