Faith-based family health care center seeking a full time Director of Quality and Risk Management to join our growing Leadership team! The Director of Quality and Risk Management is responsible for the development and implementation of all quality and risk management initiatives for the health care center. Current RN license, Bachelor's in Nursing preferred. Knowledge of HRSA and health plan quality indicators and standards, disease management and chronic disease. Previous risk management and root cause analysis experience. Thorough knowledge of regulatory compliance standards. Strong ability to implement corrective processes to improve operations and reduce risk. KEY JOB RESPONSIBILITIES Quality Responsibilities: Participates in practice performance according to HRSA UDS, insures Patient Centered Medical Home (PCMH recognition) and monitors health plan quality standards. Develop processes to improve the capture of quality metrics. Assist with the submission of the HRSA UDS reports. Responsible for the preparation of the HRSA Deeming Application, including data gathering for submission. Use of patient reports available within our EMR as well as information obtained from health plans to provide insight into strategies to close gaps in care. Develop proactive peer review process and report findings to the QI/QA Committee in conjunction with the medical and dental directors. Assist Chief Medical Officer with QI/QA Committee meetings. Risk Management Responsibilities: Responsible for reviewing and tracking Risk Management issues and reporting to the QI/QA Committee. Manages, with the assistance of the quality nurses, the Health Care Safety Zone injury/incident reporting software for tracking and trending reported events. Coordinates, with the assistance of the quality nurses, the risk management staff education in ECRI. Conduct root cause analysis with departments involved. Assess risk for ELFHCC. Advise on ways to reduce risk. Provide guidance in legal cases. Work with ELFHCC attorney to comply with all legal requests for information. Update the Patient Safety Risk Management plan as needed. Maintain Risk Management Education as required by the HRSA Deeming Application COMPENSATION: Salary range commensurate with education and experience.
