POSITION SUMMARY: Perform the duties required to assist students, parents and other community residents with Affinia Healthcare appointments. Assist uninsured students, parent, community residents and school personnel with applications for pertinent health care plans/programs, such as Medicaid/MoHealthNet, CHIPs, Health Insurance Marketplace, Gateway To Better Health Program, and other health care benefits programs available to Affinia Healthcare patients. Attend designated school district events to promote Affinia Healthcare school-based services. Hours: Full-time (Evenings and Weekends as scheduled) POSITION QUALIFICATION REQUIREMENTS Education: Requires high school diploma or equivalent. Experience: Requires one (1) year of community health, neighborhood outreach or related field. One (1) year experience working with youth and parents in a direct customer service capacity Skills and Abilities Strong verbal and written communication skills. Good organizational skills and interpersonal skills. Good computer skills including the use of word processing and e-mail software. Transportation Requirement: Must possess a valid driver's license and automobile with insurance coverage as required by the State of Missouri. License(s)/Certification(s) Required: Must participate in Exchange Marketplace training and successfully pass certification exam. CPR Certification required. Essential Functions Contacts students, parents, teachers, and other community residents to promote Affinia Healthcare School-Based services. Collaborates with other Affinia Healthcare School-based staff, as well as designated school personnel to achieve goals. Submit patient appointment requests to the appointment staff. Track and monitor kept appointments. Recruits and assists with enrollment in Gateway to Better Health, the Medicaid/MO Health Net Programs, Presumptive Eligibility, DOSA, Temporary Medicaid and utilizes the EMOMED MO Health Net provider system - for approval status. Recruits for participation in Health Fairs, Outreach drives, Chronic Disease services, Mammography screenings, special Dental activities and WIC. Distributes Affinia Healthcare approved fliers at designated schools and neighborhood sites, such as stores and homes. Enters data into the NEXTGEN system. Keeps daily logs of phone calls, school events attended, home visits, other contacts and monthly statistical reports. Conducts surveys and assists with monitoring patient/client flow in waiting area as required. Performs other duties as assigned. Marginal Functions Makes presentations on programs when needed Attends other Affinia Healthcare and community events as assigned. Attends training sessions and all meetings as required. Supervises: None NOTE: Affinia Healthcare is a Covid-19 Vaccine Mandated Employer. All employees of Affinia Healthcare are required to be vaccinated for Covid-19.
