Inspired homecare is looking for qualified, experienced Home Health Occupational Therapist for the Tempe, AZ area. Interested candidates should have experience with Home Health, be able to evaluate a patient's functional status (muscle function, endurance, visual coordination, written and verbal communication skills, self-care ability etc.) In Whole we are looking for Occupational Therapist able to coordinate, manage and deliver appropriate patient care. Job Functions/Responsibilities: Evaluate patient's functional status. Develop treatment program and goals for patients. Document in HCHB EMR software in a timely matter patients progress Participate in discharge planning. Qualifications AZ Fingerprints OT License, AZ Driver's license with valid auto insurance CPR Certification Experience with HCHB preferred but not required. Minimum 1 year home Health experience Willing to provide pre-employment physical and 2 step TB.
