Nurse Manager Job Description Department: Philadelphia FIGHT Community Health Centers Home base: John Bell Health Center Supervisor(s): Director of Nursing Full-Time, 40 hours per week, M-F; Exempt Philadelphia FIGHT Community Health Centers is a Federally Qualified Health Center (FQHC). Our health centers take a team-centered, multi-disciplinary approach and offer clinical care, access to behavioral health and substance abuse services, a wide range of educational programs, HIV counseling and testing, and youth programs. We have 4 medical health centers including our adult health centers: the John Bell Health Center and the Jonathan Lax Treatment Center, YHEP, our adolescent and young adult health center, and our Pediatric health center. We also have a satellite clinic at Broad Street Ministry. The Nurse Manager will be based at the Jonathan Lax treatment Center, which provides culturally competent, non-judgmental, high quality health care to adults. The Nurse Manager may also provide coverage for nursing colleagues working in our other health centers. The ideal candidate will take a trauma-informed and harm-reduction approach to health care. Job Responsibilities Clinical (0.40 FTE) Triage sick/emergency walk-ins and sick calls and new patients (take full medical history and determine urgency of illness) and communicate to medical providers Communicate normal and abnormal labs to patients and providers Assist with one-on-one management and teaching of complicated patients with multiple diagnoses, complicated medical regimens, and/or complex social needs Work in collaboration with the PrEP (Pre-Exposure Prophylaxis for HIV) team, to provide HIV prevention to at-risk patients, the C a Difference (Hepatitis C) team to provide access to Hepatitis C evaluation and treatment for infected patients Identify and advocate for patients needing urgent or ongoing behavioral health services and/or substance abuse treatment via collaboration with the primary provider and/or onsite Behavioral Health Consultant. Administer therapeutic injections and vaccinations and perform phlebotomy when needed Perform clinical tasks: i.e., EKG, oxygen set-up, blood glucose monitoring, urine dipsticks and pregnancy testing Perform CLIA related tasks such as running of controls Provide wound care particularly but not limited to patients with skin and soft tissue complications of injection drug use Work as a team with medical receptionists, health information specialists, medical assistants, benefits coordinators, medical case managers, pharmacy technicians, nutritionists and behavioral health specialists Demonstrate commitment to clinical care and confidentiality Administrative (0.60 FTE) Supervise Care Coordination Nurse and clinical Medical Assistants. Demonstrate commitment to clinical care and best practice by being proactive and purposeful in creating a successful patient experience. Participate in daily clinical huddle with providers, nurses and other support staff. Report and assist in communicating appropriate information on reportable diseases to oversight bodies, g., Public Health Department, AIDS Activities Coordination Office. Maintain Emergency kits, wound care kit and other specialized supplies. Interact closely with the other clinicians in the practice, Director of Adult Medicine, Practice Manager, Chief Quality Officer and Chief Medical Officer, and Senior Director of Administrative Operations by participating in regular meetings for purposes of addressing management issues and quality assurance. Participate in monthly Continuous Quality Improvement committee. Provide leadership and implementation of Team Based Care model for chronic diseases such as hypertension and diabetes. Alongside other Lax staff, participate in regular cultural competency trainings to best serve our diverse patient population. Work as a team with medical receptionists, health information specialists, medical assistants, behavioral health specialists, community health workers, practice manager, medical providers and medical case managers. Participate in monthly all health center nursing meeting and collaborate with other nurses to provide the highest level of care to our patients. Exercise good judgment in bringing any issues necessary to the attention of the Chief Quality Officer and/or Chief Medical Officer. Coordination of Care - Provide consultation and coordinate care of patients with clinic staff at the John Belll Health Center and at Clinica Bienestar. Charting - Complete all charting and documentation as needed and required by contracted funding sources, including assessments, progress notes, and administrative documentation. Billing- Complete all billing documentation as needed and required by contracted funding sources, including billing codes and required documentation. Supervision - Attend all John Bell Health Center and Clinica Bienestar team meetings, including Hepatitis C "Huddle" and attend FIGHT case conference weekly with all clinicians. Education: Bachelor's Degree Certification: Registered Nurse in the State of Pennsylvania Within 30 days of hire, complete training in Blood borne pathogens, Post exposure prophylaxis, and HIPAA regulations. Prior to shipping blood specimens, complete IATA training for shipping of hazardous substances, if applicable Requirements: General knowledge base in science, medicine, and the running of an outpatient medical clinic. Demonstrate the ability to understand the overall purpose of clinical care and importance of support staff. Excellent interpersonal skills and ability to work collaboratively with primary care staff, patients, and other behavioral health counselors. Must be able to demonstrate judgment, patience, integrity, organization, and prioritization in multi-task environment. Needs to have attention to detail, dependability and flexibility. Bilingual/ Spanish language skills preferred Salary: Competitive (Salary range is 77-93k a year depending on experience) Physical Demands: Ability to continuously stand, walk or sit. Ability to bend, climb stairs frequently. Ability to lift up to 10 pounds infrequently. Ability to squat occasionally. Work Environment: Generally works in a clinical office environment but may occasionally be required to perform job duties outside of the typical clinical setting. Note: Initial probationary period is three months. Benefits including health benefits as per employee handbook begin on the first day of work.
