To learn more about our company, please visit our website at https://www.wc-health.com/ Objective: WC Health is seeking a full time qualified Integrative Case Manager to provide services to clients with substance use disorders. WC Health is a multidisciplinary Behavioral Health Clinic looking for Case Managers to join our growing team. We are hiring case managers that will be working hands-on with clients, primarily by linking them to resources and monitoring their progress. Our Case Management team works out in the community, the Well Care Clinic, in local hospitals, in our Community Detox/Crisis Stabilization Center, our transitional housing program, at our Psychiatric Urgent Facility, in our Medically Assisted Treatment (MAT) team, with our youth program and in upcoming projects. The Crossroads Case Manager works in our Crossroads program, a long-term sober living community with wrap-around supportive services and a dynamic curriculum. Essential Functions: Assess clients' strengths and needs Develop individualized service plans Work with the multidisciplinary team to ensure client care, linking clients with other community resources. Coordinate services including health care and monitoring sufficiency of services and goal attainment; collaboration with other professionals, and group facilitation. Perform other work as required. Knowledge, Skills and Abilities: Knowledge of serious mental illness, care planning, substance abuse, psychotropic drugs, entitlement programs and community resources (Required). Experience with Medicaid documentation with an electronic medical record is preferred, however not required. Computer skills and the ability to type (Required). Position Type: Full Time hours, preference for those willing to work alternative hours. Education and Experience: Bachelor's degree in Human Services or related field and a minimum of two years of experience working with adults living with substance abuse diagnoses is preferred; or an equivalent combination of training and experience. A CADAC, the CASAT minor, or the ability to become a CADAC is preferred. Previous experience facilitating groups with this population is preferred. Additional Qualifications: Have a good driving record that must not reflect a total accumulation of six or more negative points within the past three years or a major violation within the past four years (Required). Pre-employment drug testing and FBI criminal background check (Required). Current Enrollment with Nevada Medicaid as a QBA or QMHA is preferred. Job Posted by ApplicantPro
