Homeless Response Team LINK Outreach Worker Hours: Varies Status: Full Time Reports to: HRT Director Purpose: Under the direct supervision of the Director of Crisis Teams, this position will conduct outreach efforts to locate, provide community case management, bridge housing, and determine eligibility for individuals experiencing chronic homelessness, or other populations, as identified in the community queue. Duties, Functions and Responsibilities: Provide crisis intervention that promotes wellness and recovery. Provide outreach and case management in bridge housing, to include stabilizing wrap around services. Assist clients with accessing and navigating community resources and support. Provide follow-up services to clients who have been encountered, and provide transportation to those who have accepted services. Conduct initial intake interview and client needs assessments. Maintain accurate documentation of services provided to clients, in real time (daily), into HMIS. Be able to work in remote areas (ie. Encampments, and areas not meant for human habitation). Develop and leverage relationships with first responders, hospitals, shelters, jails, homeless service providers, coordinated outreach teams and other stakeholders to locate individuals referred. Provide the verifications necessary to "certify" that a client is eligible for available housing programs or permanent housing destinations. Assist with obtaining important documents that are necessary for access to mainstream benefits, permanent housing, and other services (ie. Birth certificate, DD214, social security card, etc.) Provide ongoing, frequent check-ins on clients in the bridge housing unit. Provide a seamless, warm handoff to community housing service providers identified appropriate for a client. Assist clients with transitioning into a permanent supportive housing placement within 14 days of the opportunity becoming available. To locate and engage 75% of the clients referred by the community matcher, that are living in places not meant for human habitation. Knowledge, Skills and Abilities: Knowledge of local social service agencies and programs, as they pertain to the local homeless populations. Communicate effectively with persons from a variety of social, cultural and economic backgrounds, and possess a demonstrated understanding and sensitivity to culturally diverse populations. Effective written verbal and listening skills. Independently initiates and responds to correspondence. Knowledge of social service work, interviewing/referral techniques. Ability to maintain order and exercise appropriate judgment in crisis situations. Is flexible and can quickly adjust to shifting priorities. Organize work schedule, and budget time. Knowledge of applicable laws, statutes, ordinances and codes. Experience and understanding of homeless consumer issues. Must have the ability to understand and follow complex instructions and procedures. Analyze situations accurately and adopt an effective course of action. Ability to work flexible hours, as required by client case load. Qualifications, Education and/or Equivalent Experience: Bachelor's degree in Social Work, or High School Diploma/GED with (2) two years of experience, substantially equivalent to a bachelor's degree, or combination of education and experience. Professional experience, as required by education. Must pass a background check and pre-employment drug test. Random drug screening is performed. Must possess a valid NV driver's license, and clean DMV record. Driver History Reports are required to be submitted on a quarterly basis.
