Residential Housing (Youth and Adult Programs) : Our Residential Service Program offers a continuum of housing for the acutely and chronically mentally ill with supervised residences. Our purpose is to help those suffering from severe mental illness to live in the least restrictive setting that is compatible with their level of functioning. Position Summary : The Residential Nurse is an important part of a dynamic team of residential supervisors, program director, and direct support staff, focused on compassionate and therapeutic service and care. Participate in innovative initiatives designed to give clients appropriate information to make informed decisions about their care and treatment. Collaborate with team members to ensure medication education and administration takes place. Ensure medical documentation is accurate, timely and confidential. Serve as lead in facilitating healthy living through medical appointments, dietary support, and health education. Provide timely nursing assessments and be a leader in the care plan for our residents Qualifications : NJ RN license in good standing. A valid driver's license, use of an insured vehicle and an acceptable driving record is required for this position. Locations : Gloucester County, Cape May County Program for Assertive Community Treatment (PACT) : Program for Assertive Community Treatment (PACT) is an intensive, team-based behavioral health service that offers individualized support in the community to people with serious mental health conditions and high service needs. PACT assists individuals in their recovery from mental health conditions and in developing fulfilling lives, as well as increasing their time and engagement in community settings rather than institutions such as hospitals and jails. Position Summary : As part of a multi-disciplinary, core services team, the PACT Nurse provides nursing assessment, direct support services, and primary health care to individuals with seriously and persistently mentally ill individuals enrolled in the PACT program. The PACT Nurse will assess the medical history and current needs of the client, work with the PACT team to develop integrated medical and mental health treatment goals, and provide support and education to other team members in nursing/medical treatment and health care. Qualifications : NJ RN license in good standing. Minimum of one year of experience with individuals with serious and persistent mental illness. A valid driver's license, use of an insured vehicle and an acceptable driving record is required for this position. Location: Gloucester County Nurse-Family Partnership : Nurse-Family Partnership empowers first time moms to transforms their lives and create better futures for themselves and their babies. NFP works by having specially trained registered nurses visit first time moms-to-be in their homes, starting early in pregnancy, and continuing through child's second birthday. Position Summary : Nurse-Family Partnership (NFP) Nurse, you will visit with first-time moms in through their pregnancies until their children are 2 years old, developing enduring relationships. Your support will help your clients take control of their lives, nurture healthy children and build strong families. As a Nurse-Family Partnership nurse, you will form long-term trusting relationships with new moms to help ease them through the emotional, social and physical, challenges of becoming a parent. Being a Nurse-Family Partnership nurse allows you to be part of a supportive team helping to meet the needs of your clients while being part of a movement of caring professionals. Qualifications : NJ RN license in good standing. BSN Required, current BLS required. A valid driver's license, use of an insured vehicle and an acceptable driving record is required for this position. Location: Atlantic County, Cape May County Counseling and Wellness Center : Counseling and Wellness Center provides assessment and evaluation, individual therapy, marriage and family therapy, group therapy, psychoeducation services. Position Summary : Be a part of a dynamic multidisciplinary team including Prescribers and Therapists that focuses on compassionate, therapeutic and psychiatric services through the Counseling & Wellness Centers. Support the team in ensuring holistic care is being offered for individuals needing medical and mental health treatment. Under both general and nursing supervision, provide support to the psychiatrists and APNs efforts to stabilize individuals with serious and persistent mental illness. Administering injections, coordinating the Clozaril clinic/lab follow up, reviewing laboratory test results, providing medication education, etc. and provide input into clients' treatment plans as it relates to medication/health-related issues. Participate in innovative initiatives designed to engage communities in care and treatment. Qualifications : NJ RN license in good standing.
