Title: Population Health Nurse for Pediatrics Job Purpose: As a population health nurse, you will work collaboratively with providers and integrated team members of Health Ministries Clinic (HMC) in order to provide exceptional patient care. You will promote and restore patient's health with the use of professional nursing processes. Duties/Responsibilities: Contact patients who go to the ER and schedule follow-up visits as needed. Send referrals as needed from ER visits. Inform Pod staff of each ER visit. Review and report to pod staff the condition of admitted patients Review patients' charts who are on the CACP list and contact patient Assist with pre-visit planning, printing daily huddles for visit Update PVP flowsheet Work on care gaps DRVS - review/contact patients not seen in 1 or more years attempting to get patient scheduled Assist in scheduling mammograms when patients are in the clinic Monitor and ensure that patient's follow-up visits are scheduled Reconcile diagnosis codes Ensure abnormal VS are rechecked before the patient leaves the visit Assist with scheduling AWV Fly/Intermediate visits Additional Details: Job Type: Full-Time Schedule: Monday-Friday, 8AM-5PM Benefits: Health, Dental and Vision insurance, PTO, 401K with matching, Life insurance, Discounted wellness memberships, and more!
