Care Coordinator - Community (Temporary, Grant Funded) Real-world impact. Life-changing outcomes. CommQuest seeks a Care Coordinator-Community . Community need for services is tremendous as evidenced by the data on overdose and deaths. Converting that need in to demand requires outreach to show community members in need a path to services, to educate on what recovery services are available and to keep that education and invitation very visible. This role would support our clinical outreach coordinator (who is focused on hospitals, case managers and other professional referral sources) to do additional outreach in the recovery community and other organizations so we can work together to get more people in to recovery. CommQuest Services is one of the largest non-profit mental health, addiction recovery, and social services organizations in Northeast Ohio, working with clients from more than 34 counties in 12 facilities through more than 20 programs. We help people make positive changes in their lives through in-patient, outpatient and school-based programs, along with supportive services - from basic needs to employment assistance. Improve access to Care. Identifies potential community partners and referral sources in Stark and the surrounding counties (when appropriate, more broadly) and builds strong relationships to ensure CommQuest is top-of-mind when making referral decisions. Outreach includes recovery community, medical providers, courts, attorneys (including defense attorneys who may need resources for clients), other behavioral health agencies and other addiction recovery providers whose services CommQuest can supplement. Provide crisis intervention as needed. All other duties as assigned Schedule: Candidates much be unemployed for the past six months to qualify for grant. This is a Temporary (1 year), Full-Time position, Canton/Massillon campus locations. Minimum Qualifications: High School Diploma or GED equivalent is required Must have reliable transportation Confidence and capability to proactively meet new people who share the CommQuest mission and create connections Must have basic computer skills including MS Office Must possess the ability to work with the public in general, at time of crisis situations, and under pressure while maintaining common sense and the ability to assess the situation quickly to respond to the vulnerable population CommQuest helps. Must be able to communicate with clients, visitors, staff, etc. using compassion and understanding. Must possess excellent interpersonal skills, intermediate verbal and written communication skills, ability to show discretion when dealing with confidential information, good judgment, and organizational skills. Must be fluent in spoken and written English. Must be physically able to stand and climb steps throughout the facility frequently according to shift responsibilities. Must possess a valid driver's license, vehicle insurance, and a driving record that is in compliance with agency insurance requirements. Must pass background check in accordance with agency standards.
