The Care Coordinator serves as an administrative liaison and resource person throughout the Rehab Unit facilitating, coordinating, and overseeing the process of patient care. This role is responsible for assuring the provision of effective, high quality patient care, management of clinical nursing practice and patient care delivery, and patient education. The Care Coordinator provides support and guidance for clinical policies and procedures as well as ensures compliance with appropriate regulatory agency requirements. Clinical nursing coordinators may also provide some level of patient care. What will I do as a Care Coordinator at Saint Elizabeth? Ability to work in a dynamic unit where patient admission and discharges are high. Chart appropriately, accurately, and in a timely manner Provide, manage, and coordinate patient care and services through interpersonal contact which allows patients to attain or maintain the highest practicable physical, mental, and psychosocial wellbeing. Coordinate patient care plans and services. Exhibit excellent customer service and a positive attitude towards patients. Demonstrate dependable, regular attendance. Facilitate communication between themselves and charge nurses to ensure optimal patient outcomes. Assist charge nurses with admissions and discharges. Collaboratively working with Unit Manager to ensure superior communication and oversight of the Rehab Unit May require flexible schedule as to assist with admissions and discharges. Educates and trains nurses on procedures, policies, competencies as needed. Stays current on conditions of patients through rounding, report, reading chart... Works with Medical staff as needed. Assists in developing Care Plans Collaborates with all members of the care team. Ensures skilled documentation is appropriate. Ensures proper utilization management of skilled services and participate in skilled meetings. Care Coordinator Requirements: Graduate of Nursing Program Current and valid Rhode Island RN or LPN Certification 1 year of long-term care experience, preferred. Why choose Saint Elizabeth Community?
